Before Your Appointment
- New clients will be asked to complete a secure online intake form.
- Please arrive a few minutes early to settle in.
- Direct billing may be available depending on your insurance provider.
Cancellation Policy
A minimum of 24 hours’ notice is required to cancel or reschedule appointments. Missed appointments or short-notice cancellations may be subject to a fee.
